Q: What components are included in a patient’s report?
A: A patient’s report includes the following components:
- Criteria Set: Summary of matching criteria for the tort case using a color-coded scale (Green = match, Orange = possible/partial match, Gray = no match).
- Patient Details: Information specific to the patient.
- FILLS: An alphabetical listing of prescriptions filled by the patient, including medication details, provider and pharmacy information, dosage, supply timeline, and matching criteria.
- Medical Claims: A chronological listing of diagnosed medical conditions, including provider details, diagnosis and procedure codes, and service dates.
Q: How are medications listed in the report?
A: The prescriptions are listed in alphabetical order and each listing has the name, form and dosage of medication as well as a timeline indicating when each prescription was filled.
Q: How is provider contact information presented?
A: Provider name, address and/or phone number will be provided if that information exists with the claims data database.
Q: Is there a cost associated with this new claims data service?
A:$75 per claimant if there is a “hit”. A hit is defined as whenever any claims data is returned. If no claims data is returned then there is no charge to the law firm
Q: Do I still need to request and retrieve full records?
A: Yes, while this quick summary of claims data is helpful in identifying case support, you’ll still need to request records to properly support your case.
